IKS Health analyzed charts for 70,000 patients through a clinical chart review across four healthcare organizations. The analysis focused on several key areas within the charts: hierarchical condition category recommendations under v28, average risk adjustment factor score improvement, revenue growth from accepted recommendations, and return on investment.
IKS Health clinicians conducted a detailed analysis of the last two years of each patient’s records to suspect new diagnoses and capture previously undiagnosed, HCC-eligible chronic conditions. They then shared non-validated HCC recommendations and inferred diagnoses for the client physician’s review, with bi-directional feedback so both sides could refine the findings. Once a recommendation was made, it was assigned to the client for review and sign-off.
Across four organizations and 70,000 patients, the review uncovered conditions that had gone undiagnosed, sharpened risk scores, and returned multiples of the investment. Uncovering these previously undiagnosed conditions improved the overall quality of care for the patients.
