The prospective chart review challenge behind a growing ACO REACH program

As The Jackson Clinic participated in ACO REACH program, leadership wanted every patient seen with the most accurate and current clinical picture possible, so clinicians could treat and document conditions correctly at the point of care. The group had been running prospective chart reviews in-house, relying on three of its own physicians to review charts ahead of scheduled visits.

That internal model became difficult to sustain. It was costly to keep experienced clinicians on chart review, hard to scale as the population grew, and demanding to run at the pace the program required without compromising clinical rigor. Any suggested conditions also had to fit cleanly into the group’s existing CDI process, where clinicians review suspected codes and close them out at the next visit. A high volume of weak suggestions would have eroded clinician trust and stalled the program.

How IKS Health built a physician-led chart review foundation

The Jackson Clinic brought in IKS Health to review its ACO REACH population, and once leadership saw the quality of the work, the group replaced its internal reviewers with the IKS team.
