The Jackson Clinic is an independent multispecialty group participating in ACO REACH, serving an attributed population risk-adjusted under the CMS-HCC V28 model. As a Global Option participant with a full shared savings rate, the group carries direct accountability for both the cost and the quality of care across its population.
As The Jackson Clinic participated in ACO REACH program, leadership wanted every patient seen with the most accurate and current clinical picture possible, so clinicians could treat and document conditions correctly at the point of care. The group had been running prospective chart reviews in-house, relying on three of its own physicians to review charts ahead of scheduled visits.
That internal model became difficult to sustain. It was costly to keep experienced clinicians on chart review, hard to scale as the population grew, and demanding to run at the pace the program required without compromising clinical rigor. Any suggested conditions also had to fit cleanly into the group’s existing CDI process, where clinicians review suspected codes and close them out at the next visit. A high volume of weak suggestions would have eroded clinician trust and stalled the program.
The Jackson Clinic brought in IKS Health to review its ACO REACH population, and once leadership saw the quality of the work, the group replaced its internal reviewers with the IKS team. IKS Health deployed its physician-led prospective clinical chart review service across the full roster, with a twenty-four month lookback on every chart. Recommendations were delivered in the clinic’s own output format and printed directly onto the MIPS forms clinicians already use, with no re-keying or reformatting. The two teams held weekly, monthly, and quarterly business reviews, and The Jackson Clinic scored every recommendation through its own monthly feedback, keeping both sides working from the same source of truth.
Over roughly nine thousand six hundred charts across 5,358 unique lives, reviewed between February and December 2025, the partnership improved both care and financial performance.
